Minnesota Instructions — Request for Subpoena is a legal document issued by a court in Minnesota to require a person to appear at a certain time and place to give testimony or to produce documents. This document may also be used to collect evidence for a court proceeding. There are two types of Minnesota Instructions — Request for Subpoena: a Subpoena Duces Cecum and a Subpoena ad Testificandum. A Subpoena Ducks Cecum is a court order to bring specified documents or records to court for use as evidence. A Subpoena ad Testificandum is a court order to appear in court to testify as a witness. The Minnesota Instructions — Request for Subpoena must include the name of the court issuing the subpoena, the date of the hearing, the name of the person(s) required to appear, the address of the court, and a description of the documents or testimony required. It must also contain the signature of the court clerk or judge. The person receiving the Minnesota Instructions — Request for Subpoena must serve the document, in person, to the person required to appear in court. The person must also fill out an Affidavit of Service form to confirm that the document was properly served.
