Minnesota Affidavit of Service By Mail is a form that is used in the state of Minnesota to document the successful service of legal documents by mail. This form is typically used when the recipient of legal documents is not able to be personally served. There are two types of Minnesota Affidavit of Service By Mail: Regular and Certified. The Regular Minnesota Affidavit of Service By Mail requires the server to list the date, time, and place of service, as well as the recipient's name and address. The server must also certify that they delivered a copy of the legal documents by mail to the recipient. The Certified Minnesota Affidavit of Service By Mail requires the same information as the Regular version but also requires the server to obtain a Certificate of Mailing from the United States Postal Service. This certificate confirms that the documents were sent on the date indicated in the affidavit.
