The Minnesota Affidavit of Personal Service is a legal document used to certify that court papers have been served in accordance with the law. There are two types of Minnesota Affidavit of Personal Service: the Affidavit of Service by Mail and the Affidavit of Service by Personal Delivery. The Affidavit of Service by Mail is used when court documents are served by mail; it must include the name and address of the person served, a description of the documents served, a statement that the documents were mailed, and the date the documents were mailed. The Affidavit of Service by Personal Delivery is used when court documents are served in person; it must include the name and address of the person served, a description of the documents served, a statement that the documents were handed directly to the person served, and the date the documents were served. Both Affidavits must be signed by the person who served the documents and notarized.
