Minnesota Petition For Appointment of Guardian or Conservator is a legal document which is used to request the appointment of a guardian or conservator for an individual who is unable to make decisions for themselves due to a mental or physical disability. There are two types of Minnesota Petition For Appointment of Guardian or Conservator: (1) Petition for Appointment of Guardian of Incapacitated Person; and (2) Petition for Appointment of Conservator of Protected Person. The Petition for Appointment of Guardian of Incapacitated Person is used when the individual is unable to take care of themselves and requires the assistance of a guardian to make decisions on their behalf. The Petition for Appointment of Conservator of Protected Person is used when the individual is unable to manage their own financial affairs and requires the assistance of a conservator to manage their finances and property. Both petitions require the petitioner to provide details about the incapacitated or protected person, including their name, address, age, and disability, and must be accompanied by a medical evaluation. The petition must also be signed by the petitioner(s).
