There are two types of Minnesota Affidavit of Personal Service: (1) Affidavit of Service by Mail, where the documents are mailed to the person to be served and the affidavit is filled out by the individual who mailed the documents; and (2) Affidavit of Service by Delivery, where the documents are hand-delivered to the person to be served and the affidavit is filled out by the individual who delivered the documents.
