The Minnesota R-2 Rehabilitation Plan is a state-sponsored program created to help individuals with a declared physical disability live independently in their own homes. The program is managed by the Minnesota Department of Human Services and provides financial assistance to help cover the costs of home modifications, such as ramps, stair lifts, bathroom modifications, and other assistive devices. The program also offers a range of other services to help individuals with physical disabilities gain access to employment, education, and other activities. There are three types of Minnesota R-2 Rehabilitation Plans: the Home Modification Plan, the Assisted Living Plan, and the Home and Community Based Services Plan. The Home Modification Plan provides financial assistance to help cover the costs of home modifications, such as ramps, stair lifts, bathroom modifications, and other assistive devices. The Assisted Living Plan provides assistance to help cover the cost of an assisted living facility and related services. The Home and Community Based Services Plan provides funding for a wide range of services, such as home health services, transportation services, and community activities.
