Minnesota Rehabilitation Consultation Reports (Mrs) are reports used by the Minnesota Department of Human Services (DHS) to assess and develop plans for individuals with disabilities. Mrs assist in making decisions about the type of services a person needs to help them live a more independent and productive life. The reports are completed by an interdisciplinary team of qualified professionals who evaluate the individual's needs, abilities, and goals. There are three types of Mrs: Initial Consultation, Follow-up Consultation, and Expedited Consultation. An Initial Consultation assesses the individual's current level of functioning and makes recommendations for services to help them reach their goals. Follow-up Consultations are used to track progress and make adjustments to the individual's plan as needed. An Expedited Consultation is used when a determination of eligibility needs to be made quickly. Mrs provide an in-depth assessment of the individual's needs, abilities, and goals and provide the basis for determining eligibility for services and supports. The reports are an important tool in helping the individual reach their maximum potential.
