Description Age Of Consent Us States

Any individual of sound mind who is at least eighteen years of age,
or under eighteen years of age with parental consent, may give all or any
part of his body for any statutorily allowed purpose. This is a state specific form which designates the body parts and organs that a minor who is emancipated, or able to apply for a driver's license, or unemancipated and has parental consent, and wishes to donate at the time of death. This form provides for the necessary parental consent to effectuate such a gift. A gift of all or part of the body may be made by will or by a document other than a will.
