The Missouri Adversary Proceeding Cover Sheet is a form that must be filed with the clerk of the bankruptcy court when initiating an adversary proceeding (a lawsuit within a bankruptcy case). It is required to be completed and filed by both the plaintiff and the defendant in addition to the complaint or other initial pleading. The cover sheet must include the caption, the names of all parties, the adversary proceeding title, the assigned judge, the assigned trustee, the adversary proceeding number, and the appropriate filing fee. The Missouri Adversary Proceeding Cover Sheet comes in two types: the Missouri Adversary Proceeding Cover Sheet for General Adversary Proceedings, and the Missouri Adversary Proceeding Cover Sheet for Adversary Proceedings Involving Discharge or Denial of Discharge.
