The Missouri Neutrals Application Form is a document used to register and apply to become a neutral in the state of Missouri. The form is available in three different types: Mediator, Arbitrator, and Special Master. Each type of application requires different information and qualifications. For the Mediator application, the form requires background information such as name, address, phone number, education, and experience. It also requires a resume, a copy of the applicant’s professional license, and a signed and notarized affidavit of compliance. The Arbitrator application form requires similar information to the Mediator application, as well as a copy of the applicant’s arbitration certificate. It also includes a signed and notarized affidavit of compliance, and a signed and notarized affidavit of agreement with the Missouri Arbitrators Code of Conduct. The Special Master application requires additional information such as a list of the applicant’s professional affiliations and references. It also requires a signed and notarized affidavit of agreement with the Missouri Special Masters Code of Conduct. All three Missouri Neutrals Application Forms require a non-refundable application fee.
