This certificate discloses the organization’s name, address, date of formation, purpose(s), and the name and address of its registered agent. It also provides information regarding the organization’s officers, directors, members, and partners. The certificate must be signed by an authorized representative of the organization and notarized. There are two types of Missouri Disclosure of Organizational Interests Certificate: Initial certificate and Renewal certificate. The initial certificate must be filed within 30 days of acquiring an interest in real estate or tangible property in Missouri, while the renewal certificate must be filed annually.
