The Missouri Fingerprint Information Form is an official document used by the Missouri State Highway Patrol (MHP) for the purpose of collecting and processing fingerprints. It is a mandatory document that must be completed when requesting fingerprint-based criminal history record checks. There are two types of Missouri Fingerprint Information Form: the Non-Criminal Applicant Form and the Criminal Applicant Form. The Non-Criminal Applicant Form is used for non-criminal related purposes such as employment background checks, pre-employment screening, and licensing. The Criminal Applicant Form is used for criminal-related purposes such as criminal investigations, arrests, and court proceedings. Both forms require the applicant to provide their personal information, such as name, date of birth, address, and Social Security Number. Additionally, the Non-Criminal Applicant Form requires the applicant to provide their employer's name and address. Once the form has been completed, the applicant must provide their fingerprints at a designated fingerprinting station. The fingerprints will then be sent to the MHP for processing and comparison with their fingerprint database.
