The Missouri Monthly Supervision Report is a report submitted by Missouri Probation and Parole Officers to track the progress of individuals on probation or parole. It contains information regarding the individual's supervision activities, such as the number and type of contacts made with the individual, any violations of supervision, and any positive or negative changes in the individual's behavior. The report also includes details of any community service activities or employment that the individual has completed. There are three types of Missouri Monthly Supervision Reports: the Standard Report, the Supervised Living Report, and the Intensive Supervision Report. The Standard Report is the most common type and includes details such as the individual's name, address, and birthdate; the type of supervision (i.e. probation or parole); the date of the supervision offense; the date of the start of supervision; the date of the end of supervision; the number of contacts made with the individual; any violations of supervision; and any positive or negative changes in the individual's behavior. The Supervised Living Report is similar to the Standard Report but also includes information about the individual's living arrangements, such as whether they are living in a halfway house or with family. The Intensive Supervision Report is the most comprehensive and includes information about the individual's activities on a daily basis, such as the number of hours spent in school or work, the type of activities engaged in, any medications taken, and any contacts with law enforcement.
