The Missouri Affidavit of Credit by Custodial Parent is a legal document used by the custodial parent of a minor child to establish a claim for reimbursement of child support payments made to the non-custodial parent. This document is used when a court order requires the custodial parent to provide proof of payment to the non-custodial parent. The affidavit can also be used to establish or modify a child support order. The Missouri Affidavit of Credit by Custodial Parent can take two forms: the Short Form and the Long Form. The Short Form is used for payments made between October 1, 2018, and the present. The Long Form is used for payments made prior to October 1, 2018. Both forms require the custodial parent to provide information about the payments made, such as the amount, date, and method of payment. The custodial parent must also sign and date the affidavit and have it notarized.
