The Missouri Application of Unmarried Minor Child(men) for Refusal of Letters (Sec.473.090 RSM)- Automated Form is an automated form used to legally refuse the appointment of legal guardianship for unmarried minor children in the state of Missouri. This form is typically used when the parent of the minor child wishes to retain full legal custody of the child. The form must be signed by the parent and all other interested parties, and submitted to the court for review. The form consists of various sections which must be completed in order for it to be accepted by the court. These include: • The child's name and birthdate • The parent/guardian's name, address, and contact information • A statement that the parent/guardian is refusing the appointment of legal guardianship • A statement that the parent/guardian understands the legal consequences of signing the form • A statement that the parent/guardian has read and understands the form • The signature of the parent/guardian, as well as any other interested parties • The signature of a notary public Once completed, the form must be filed with the court in order for it to be legally binding.
