The Missouri Certificate of Service of Required Documents Pursuant To Rule 68.4.1(4) (Sample and Form) is a document issued by the Supreme Court of Missouri that must be completed by a legal practitioner when serving certain documents in a civil case. This document certifies that the required documents have been served in accordance with Rule 68.4.1(4) of the Missouri Rules of Civil Procedure. There are two types of Missouri Certificate of Service of Required Documents Pursuant To Rule 68.4.1(4) (Sample and Form): the Certificate of Service of Original Documents and the Certificate of Service of Copies of Documents. The Certificate of Service of Original Documents must be signed and dated by the legal practitioner and include the name and address of the person or entity to whom service was made, the date of service, the method of service, and the signature of the legal practitioner. The Certificate of Service of Copies of Documents must be signed and dated by the legal practitioner and must include the same information as the Certificate of Service of Original Documents, as well as the name and address of the person or entity to whom the copies were sent.
