The Missouri Complaint Information Sheet is a document used by the Missouri Department of Insurance, Financial Institutions & Professional Registration (DIP) to investigate complaints against insurance companies, financial institutions, and professional entities. The sheet includes information about the complainant and the issue. It is used to collect details, such as the complainant’s name, address, telephone number, name of the entity complained against, and the nature of the complaint. There are three types of Missouri Complaint Information Sheet. The first is the Consumer Complaint Information Sheet, which is used to investigate complaints against insurance companies. The second is the Financial Institution Complaint Information Sheet, which is used to investigate complaints against banks, credit unions, and other financial institutions. Lastly, the Professional Entity Complaint Information Sheet is used to investigate complaints against entities that provide professional services, such as attorneys and doctors.
