The Missouri Prevailing Wage Complaint Form is a legal document used to report any violations of the state’s prevailing wage law. This form is available to anyone who believes an employer has failed to pay the legally required wage rate for a particular job or project. It is also used to report any issues with overtime pay, deductions, and/or benefits. The Missouri Department of Labor and Industrial Relations (DOLOR) oversees the prevailing wage complaint process. There are three types of Missouri Prevailing Wage Complaint Forms: the Initial Complaint Form, the Employer Response Form, and the Wage Complaint Appeal Form. The Initial Complaint Form is used to report a violation of the prevailing wage law. The Employer Response Form is used by the employer to respond to the complaint and provide evidence of compliance with the law. The Wage Complaint Appeal Form is used by the complainant if they are dissatisfied with the response from the employer.
