The Missouri Quarterly Wage Report Continuation Sheet is a form used by employers in the state of Missouri to report additional wages to the Missouri Department of Revenue. The form is used to report wages that exceed the maximum number of employees or wages that exceed the limits for the standard quarterly wage report. This form is used in conjunction with the Missouri Quarterly Wage Report. The two types of the Missouri Quarterly Wage Report Continuation Sheet are the Employer's Quarterly Wage Report Continuation Sheet and the Combined Quarterly Wage Report Continuation Sheet. The Employer's Quarterly Wage Report Continuation Sheet is used to report additional wages for employers with more than 25 employees and/or wages that exceed the limits for the standard quarterly wage report. The Combined Quarterly Wage Report Continuation Sheet is used to report additional wages for employers with more than 25 employees and/or wages that exceed the limits for the standard quarterly wage report, but for employers that are combined with other employers in the same quarter.
