The Montana Caretaker Relative's Educational Authorization Affidavit is a legal document used to give a caretaker relative the legal authority to make educational decisions on behalf of a minor child. The Caretaker Relative must be approved by the Montana Department of Public Health and Human Services in order to gain this authority. There are two types of Montana Caretaker Relative's Educational Authorization Affidavit. The first is the Caretaker Relative Authorization Affidavit, which is used to gain authority to make educational decisions on behalf of a minor child. This includes decisions related to enrollment, attendance, discipline, and access to educational records. The second is the Caretaker Relative Educational Power of Attorney, which is used to give a Caretaker Relative the legal authority to make decisions regarding the education of a minor child while the child is in the custody of the Caretaker Relative. This authority includes the ability to sign educational documents, such as enrollment forms and permission slips, on behalf of the minor child. Both of these documents must be signed by the Caretaker Relative, the minor child's parent or guardian, and a witness. The Caretaker Relative must also provide a valid Montana driver's license or other proof of identity. Once the document is completed and signed, it must be notarized and filed with the local Montana Department of Public Health and Human Services.
