North Carolina Notice of Correction of SSN is a form issued by the North Carolina Division of Employment Security (DES) to employers for correcting an employee’s Social Security Number (SSN). This form is used to update an employee’s SSN in the employer's records and the DES records. There are two types of North Carolina Notice of Correction of SSN: Form NC-NOC-SSN-R-01 and Form NC-NOC-SSN-R-02. Form NC-NOC-SSN-R-01 is used when the employee has a valid SSN, but the wrong SSN has been reported to the DES. It is also used to update the employee’s SSN in the DES records. Form NC-NOC-SSN-R-02 is used when the employee does not have a valid SSN, or the employee has a valid SSN but is unable to provide the SSN to the employer. This form is used to request a new SSN to be issued to the employee by the Social Security Administration. Both forms require the employer to provide the employee’s name, date of birth, and address. After completing the form, the employer should submit it to the DES.
