North Dakota Capability Assessment is a program designed to provide a comprehensive evaluation of an individual’s physical, cognitive, and psychological abilities. This assessment is conducted to determine the degree of independence and functioning of an individual in their daily activities. The assessment includes components of physical, cognitive, and psychological functioning, and provides a snapshot of an individual’s functioning in the community. It is used to determine the level of support and services needed for an individual to remain independent and functioning in their home and community. North Dakota Capability Assessment includes three main types of assessments: the Functional Assessment, the Physical Assessment, and the Cognitive Assessment. The Functional Assessment evaluates an individual’s ability to complete daily tasks such as dressing, grooming, meal preparation, and housekeeping. The Physical Assessment focuses on an individual’s physical abilities such as mobility, strength, coordination, and balance. The Cognitive Assessment evaluates an individual’s abilities in areas such as memory, judgment, problem-solving, and communication. All three assessments provide a comprehensive overview of an individual’s functioning in the community.
