North Dakota Dentist Report of Injury is a form that provides dental professionals in North Dakota with the opportunity to report all injuries that occur in their practices. This includes any accidental or intentional damage to teeth, gums, or other oral tissue resulting from the practice of dentistry. The report must be completed and submitted within 14 days of the injury and must include the name of the patient, the type of injury, and the date it occurred. There are three types of North Dakota Dentist Report of Injury: Patient Injury Report, Employee Injury Report, and Near Miss Report. The Patient Injury Report is used to report any injury sustained by a patient while under the care of the dental professional. The Employee Injury Report is used to report any injuries sustained by a dental professional or employee while in the practice. The Near Miss Report is used to report any incident that could have resulted in an injury, but did not.
