Nebraska Debtor Certification (Chapter 13 — Appendix M) is a document used to regulate debtors in bankruptcy proceedings in the state of Nebraska. This form is used to certify that debtor(s) have completed the required financial management course, as mandated by the U.S. Bankruptcy Code, prior to filing a Chapter 13 bankruptcy petition. The form must be signed and notarized before filing with the court. There are three types of Nebraska Debtor Certification (Chapter 13 — Appendix M): Nebraska Debtor Certification of Financial Management Course (for individuals); Nebraska Debtor Certification of Financial Management Course (for married couples filing jointly); and Nebraska Debtor Certification of Financial Management Course (for married couples filing separately).
