The Nebraska Uniform Termination Notice for Securities Industry Registration Instructions (Form U5) is a form used to terminate an individual’s registration with the Nebraska Department of Banking and Finance. This form is used when an individual is or was employed as an associated person in the securities' industry. It is also used when an individual has been approved as an associated person but has not yet begun working in the securities' industry. The form is used to provide written notice to the Nebraska Department of Banking and Finance of the termination of registration of the individual. The Nebraska Uniform Termination Notice for Securities Industry Registration Instructions (Form U5) includes two types of forms: the Broker-Dealer/Agent Termination Notice (Form U5-B) and the Investment Advisor Representative Termination Notice (Form U5-I). The Broker-Dealer/Agent Termination Notice (Form U5-B) is used to terminate the registration of a broker-dealer or agent. The Investment Advisor Representative Termination Notice (Form U5-I) is used to terminate the registration of an investment advisor representative. Both forms require the individual to provide information such as name, address, and date of termination, as well as details about the termination of registration.
