The Nebraska Affidavit and Application to Proceed In Form Paupers (English) is a document used by the state of Nebraska to allow a person to represent themselves in a court case without having to pay the legal fees. It is also known as a ‘pauper’s affidavit’. The affidavit must be filled out by the individual requesting the waiver of fees and must include information about their financial status and ability to pay the legal fees. The affidavit must be signed by the individual under oath and notarized by a notary public. There are two types of Nebraska Affidavit and Application to Proceed In Form Paupers (English). The first is a general form that can be used for a variety of reasons and the second is a specific form used when the individual is requesting a waiver of fees due to a disability or inability to pay.
