The Nebraska Child Support Termination Application and Affidavit is a legal document used to terminate child support payments in the state of Nebraska. It is used when a parent has fulfilled their obligation to make child support payments, or when the child has turned 18 and is no longer a minor. The form is available online and can be filed in court or with the Nebraska Department of Health and Human Services. There are two types of Nebraska Child Support Termination Application and Affidavit: the Original Affidavit and the Certified Copy Affidavit. The Original Affidavit is filled out by the parent who is responsible for making the child support payments and must be notarized before submitting it to the court or the Nebraska Department of Health and Human Services. The Certified Copy Affidavit is used when the original has already been filed and requires the signatures of both parents. The Nebraska Child Support Termination Application and Affidavit must be completed in full and submitted to the court in order to terminate the child support payments.
