The New Hampshire Prisoner Litigation Reform Act (PLEA) Consent Form is a document that must be signed by inmates in New Hampshire prior to filing a lawsuit against the state. The form is intended to ensure that inmates are aware of their rights and the applicable regulations regarding prisoner litigation. The form outlines the rules and procedures that must be followed when filing a lawsuit, as well as the consequences of not adhering to the rules. There are three types of New Hampshire Prisoner Litigation Reform Act Consent Forms: the New Hampshire Inmate Consent Form, the New Hampshire Inmate Notification Form, and the New Hampshire Grievance Procedure Form. The Inmate Consent Form must be signed and dated prior to filing a lawsuit against the state; the Inmate Notification Form must be completed and returned to the state before the court will consider a lawsuit; and the Grievance Procedure Form must be filled out and filed with the court before a grievance can be filed.
