This form must be completed and submitted by a designated partner of the LLP. The form is available in three versions, depending on the type of LLP: 1. New Hampshire Form LLP-4 Withdrawal of NH Registration for Domestic LLP: This form is used for the withdrawal of a domestic LLP registered in New Hampshire. 2. New Hampshire Form LLP-4 Withdrawal of NH Registration for Foreign LLP: This form is used for the withdrawal of a foreign LLP registered in New Hampshire. 3. New Hampshire Form LLP-4 Withdrawal of NH Registration for Foreign Qualified LLP: This form is used for the withdrawal of a foreign qualified LLP registered in New Hampshire. The form requires the LLP’s name, state of organization, state of registration, and the date of registration. It also requires the signature of a designated partner of the LLP, as well as the signature of the Secretary of State. After the form is completed and submitted, it will be processed by the Secretary of State and the LLP will be officially withdrawn from registration.
