The New Hampshire Exception Affidavit of the Child Impact Program Attendance is a document that is used for students in the State of New Hampshire who wish to be exempt from the state's mandatory school attendance laws. The affidavit is a form that must be completed by the student's parent or guardian to request an exemption from the attendance requirement. The affidavit must be signed by both the student and the parent/guardian and submitted to the school district. There are two types of New Hampshire Exception Affidavit of the Child Impact Program Attendance: (1) Exemption from School Attendance due to Religious Reasons; and (2) Exemption from School Attendance due to Other Reasons. The Exemption from School Attendance due to Religious Reasons form must include a statement from the student's religious leader attesting to the student's religious beliefs and practices that conflict with attending school. The Exemption from School Attendance due to Other Reasons form requires the parent or guardian to provide a detailed explanation as to why the student should be allowed to be exempt from the attendance requirement.
