There are three types of New Hampshire CASA/GAL Cover Sheet: the Initial Report Cover Sheet, the Continuation Report Cover Sheet, and the Final Report Cover Sheet. The Initial Report Cover Sheet is used when a volunteer is first appointed to work with a child and their family. The Continuation Report Cover Sheet is used to document any additional visits or activities that have occurred since the initial report. The Final Report Cover Sheet is used to summarize the volunteer's activities and the results of their investigation.
