The New Hampshire Report of the Guardian of the Person — Min-filingng Only, is a legal document that is used for minors who are under the guardianship of another person. This document is filed in the court system and provides information about the minor’s health, education, and other personal matters. The document must be signed by the guardian and must include all relevant information about the minor. The document must be filed with the court system within 21 days of the appointment of the guardian. There are two types of New Hampshire Report of the Guardian of the Person — Min-filingng Only: 1) An initial report and 2) An annual report. The initial report must be completed within 30 days of the appointment of the guardian and the annual report must be completed once per year. Both reports must provide an accurate and up to date description of the minor’s health, educational, and other personal matters.
