New Jersey Information Sheet For Ancillary Administration (NISSAN) is a set of documents used by the New Jersey Department of Human Services (DHS) to provide information about an individual’s health care needs. The documents include information about the individual’s medical history, current medical condition, and the type of care that is needed and appropriate for them. NISSAN can also include information about the individual’s financial and legal resources, such as their Social Security Number, assets, and debts. There are two types of NISSAN: the Ancillary Services Information Sheet (BASIS) and the Long Term Care Information Sheet (LT CIS). The BASIS provides information about the individual’s current medical condition, current medications, and any ancillary services that they are receiving. The LT CIS provides information about the individual’s long-term care needs, including the type of care they are receiving, the amount of care they need, and any additional services that are needed.
