New Jersey Guardian Consent Minor (also referred to as CCSMSM”) is a legal concept that allows parents/guardians to give consent for their minor child to receive medical care or treatment without the presence of the child’s legal guardian. This is done when the parent/guardian is not available to give consent in person or if the parent/guardian believes it is in the child’s best interests to receive the medical care/treatment. There are two types of New Jersey Guardian Consent Minor: Standard CCSM and Expedited CCSM. Standard CCSM requires that the parent/guardian fill out a form in which they provide a detailed description of the medical care/treatment, the name of the physician/healthcare provider, and the signature of the parent/guardian. Expedited CCSM allows a parent/guardian to give verbal consent for medical care/treatment and does not require the completion of a form. In either case, the consent must be approved by a court.
