There are three types of New Jersey Confidential Litigant Information Sheets: the General Litigant Information Sheet, the Domestic Violence Litigant Information Sheet, and the Special Litigant Information Sheet. The General Litigant Information Sheet collects information such as name, address, phone number, social security number, date of birth, and other contact information. The Domestic Violence Litigant Information Sheet collects the same information as the General Litigant Information Sheet, however, it also includes a statement of domestic violence allegations. The Special Litigant Information Sheet is used in cases where the litigant is a minor, has a disability, or is a victim of identity theft.
