The New Jersey EEO Formal Complaint Form is a document used by employees of the State of New Jersey to report allegations of discrimination in the workplace based on race, color, religion, sex, national origin, age, marital status, disability, sexual orientation, gender identity or expression, genetic information, veteran status, or any other characteristic protected by applicable law. The form is used to formally initiate a complaint of discrimination or harassment and is administered by the New Jersey Division on Civil Rights (DCR). There are two types of this form: the initial complaint form and the response complaint form. The initial complaint form is used to initiate a discrimination complaint and outlines the employee’s rights and responsibilities in the process. The response complaint form is used by the employer to respond to the employee’s complaint and is typically due within 30 days of receipt.
