The New Jersey Appellate Criminal Case Information Statement (ACID) is a form used in criminal appeals in New Jersey. The ACID must be filled out completely and accurately and submitted to the appellate court prior to the appeal. The New Jersey ACID is divided into four parts. Part I includes information on the parties to the criminal case, including the name and address of the appellant, the name and address of the respondent, the name of the court where the trial was held, the county where the trial was held, the date of the trial court’s decision, and the criminal charge and sentencing information. Part II includes information on the trial court’s decision, including the court’s decision, written opinions, and transcripts. Part III includes information on the issues raised by the appeal, such as the legal basis of the appeal, the arguments raised, and the evidence presented. Part IV includes information on the relief requested, such as the requested relief, the standard of review, and the applicable law. There are two types of New Jersey Appellate Criminal Case Information Statements: the standard form and the simplified form. The standard form is used in all criminal appeals, while the simplified form is used in appeals involving certain minor offenses or when the appellant is a pro SE litigant.
