The New Jersey Case Information Statement (CIS-LP) is an official form used by the state of New Jersey to provide financial information to the court. It is used for divorce, dissolution of civil unions, child support, alimony, and other family law matters. There are three types of New Jersey Case Information Statement (CIS-LP): the CIS-LP, the CIS-LP-C (Custody), and the CIS-LP-A (Alimony). The CIS-LP is the standard form which all parties must complete. The CIS-LP-C and CIS-LP-A are supplemental forms which must be completed in addition to the CIS-LP. The CIS-LP requires individuals to provide detailed information about their current assets, income, debts, and other related financial information. This information is used by the court to determine a fair and equitable resolution for the parties involved in the divorce, dissolution of civil unions, or other family law matter.
