The New Jersey Comprehensive Accounting Form (Guardianship) is a form used by guardians in the state of New Jersey to report financial activity and transactions in the estate of a protected person. This form is used to provide an accounting of all assets, liabilities, receipts, disbursements, and distributions of the protected person’s estate. It is also used to report any other transactions or activities related to the guardianship. There are two types of New Jersey Comprehensive Accounting Form (Guardianship): the Annual Comprehensive Accounting Form (for guardianship that are open for more than one year) and the Final Comprehensive Accounting Form (for guardianship that are closed). Both forms require the guardian to provide detailed information about the protected person's assets, liabilities, income, expenses, and other transactions. The form is then reviewed and approved by the court and submitted to the Office of the Public Guardian.
