The telephone form requires the recipient to call the New Jersey Family Support Payment Center and provide the relevant information over the phone. All three forms require the recipient to provide personal information such as name, address, and bank account details. The bank account must be in the recipient’s name and must be a U.S. financial institution. The recipient must also provide the name and contact information of the mayor's employer. Once the form is completed, it should be sent to the New Jersey Family Support Payment Center for processing. The form is valid for up to two years or until the recipient requests a change or cancellation.
