New Mexico HIPAA — Authorization to Disclose Protected Health Information — Medical Records is a form used to allow a patient to authorize the release of their protected health information (PHI) from their medical records. It is used to grant permission to authorized individuals or entities to access a patient’s records for a specified purpose and for a limited period of time. The form must be completed and signed by the patient or their legally authorized representative in order for the disclosure of the PHI to be allowed. There are two main types of New Mexico HIPAA — Authorization to Disclose Protected Health Information — Medical Records: the Standard HIPAA Authorization form and the New Mexico HIPAA Authorization to Disclosure Medical Record form. The Standard HIPAA Authorization form is used to authorize the release of records to any person or organization, and the New Mexico HIPAA Authorization to Disclosure Medical Record form is used to authorize the release of records to specific individuals or entities. Both forms must be completed and signed by the patient or their legally authorized representative in order for the disclosure of the PHI to be allowed.
