Nevada Cremation Consent is a form that is required to authorize the cremation of a deceased person in the state of Nevada. It is a legally binding document that must be filled out and signed by the authorized person(s) in order for the cremation to proceed. Generally, it is the legal next-of-kin that is responsible for completing the form and providing authorization for the cremation. The form must include the name of the deceased, the name of the person authorizing the cremation, their relationship to the deceased, the name of the funeral home or crematory that will be performing the cremation, and the signature of the authorized person(s). There are two types of Nevada Cremation Consent: a standard form that is used for all cremations and an advanced form that is used when additional services or preparations are necessary.
