The Nevada Affidavit of Service is a form used to prove that legal papers were properly served to a recipient. It is also known as a proof of service form. It is a sworn statement by a server, or an individual who delivers or serves legal documents, that the documents have been served on the intended recipient in accordance with Nevada law. The form must be signed in front of a notary public. There are two types of Nevada Affidavit of Service: one for personal service and one for substituted or constructive service. For personal service, the documents must be delivered to the recipient personally. For substituted or constructive service, the documents may be delivered to a third party, such as a family member, office mate, or roommate, or they may be left at the recipient’s last known address, or they may be sent by mail. The server must complete the form with all the required information and sign it in the presence of a notary public.
