Nevada Confidential Information Sheet — NO CHILDREN is a form that is used to protect the confidential information of a minor who is not a party to a legal proceeding in the state of Nevada. It is a document that is signed by the court, a guardian ad item, or other responsible party prior to the minor's participation in any proceedings that involve confidential information. The form is used to ensure that the minor's information is not disclosed to anyone without the consent of the minor, the guardian ad item, or the court. There are three types of Nevada Confidential Information Sheet — NO CHILDREN: the Minor's Confidential Information Sheet, the Guardian Ad Item's Confidential Information Sheet, and the Court's Confidential Information Sheet.
