The Nevada Request for Archived Bankruptcy Cases (Individual) is a necessary form for individuals to submit to the court in order to receive a copy of an archived bankruptcy case. The form is used to request a case from the Nevada Bankruptcy Court's archives, which contains documents from cases that have been closed and dismissed. There are two types of Nevada Request for Archived Bankruptcy Cases (Individual): Voluntary and Involuntary. The Voluntary Request form is used by a debtor to request a copy of their own archived bankruptcy case. The Involuntary Request form is used by a creditor to request a copy of an archived bankruptcy case involving a debtor who does not have the ability to make the request themselves. Both forms require the name of the debtor, the case number, and the date of bankruptcy filing. Additionally, the Voluntary Request form requires the debtor's address and the Involuntary Request form requires the name and address of the creditor making the request.
