The High School Transcript Order Form & Instructions is used to request an official transcript of high school academic records from the Nevada Department of Education. The form requires the individual to provide personal information such as their full name, date of birth, social security number, and address. The form also requires the individual to provide the name and address of the college or other institution that they would like the transcript sent to. The College Transcript Order Form & Instructions is used to request an official transcript of college academic records from the Nevada Department of Education. The form requires the individual to provide personal information such as their full name, date of birth, social security number, and address. The form also requires the individual to provide the name and address of the college or other institution that they would like the transcript sent to. Additionally, the form requires the individual to provide their college student ID number and the number of official transcripts being requested. Once the Nevada Transcript Order Form & Instructions have been completed, the individual must submit the form and a check or money order for the applicable fee to the Nevada Department of Education.
