The Nevada Officer's Statement (NRS 80.030) is a form required by state law that must be signed by a Nevada law enforcement officer or other authorized individual. It is used to certify that a criminal offense has been committed and witnessed, or that a criminal investigation has been conducted. This form is required to be completed and submitted to the District Attorney's Office before a criminal case can be filed. The form is also used to document evidence and statements made by witnesses in criminal investigations. There are three types of Nevada Officer's Statements: 1. Arrest Statements (NRS 80.030(1)) which certify that an arrest was made and the probable cause for the arrest. 2. Investigation Statements (NRS 80.030(2)) which document the investigation conducted by the officer and any evidence gathered. 3. Witness Statements (NRS 80.030(3)) which document the statements made by witnesses during an investigation.
