The Nevada Confidential Medical or Educational Information Sheet is a document used by the Nevada Department of Education to protect the privacy of students and their families. It is a form that must be filled out by the parent or legal guardian of a student in order to ensure the confidentiality of the student’s medical or educational records. The form contains information such as the student’s name, date of birth, address, contact information, health insurance information, medical history, educational history, and any other relevant information. There are three types of Nevada Confidential Medical or Educational Information Sheet: the Student Information Sheet, the Parent/Guardian Information Sheet, and the Physician Information Sheet. The Student Information Sheet collects personal information and medical and educational history from the student. The Parent/Guardian Information Sheet collects information about the student’s parent/guardian, such as their name, contact information, and relationship to the student. The Physician Information Sheet collects information from the student’s physician, such as the doctor’s name, contact information, and any medical or educational recommendations.
