The Nevada Consent and Release Form for Fingerprinting and Criminal History Review is a form used by employers, licensing agencies, and governmental entities to obtain an individual’s authorization for fingerprinting and criminal history review. The purpose of the form is to provide the entity with consent to obtain an individual’s criminal history information from the Nevada Department of Public Safety (NPS) and/or the Federal Bureau of Investigation (FBI). There are two types of Nevada Consent and Release Form for Fingerprinting and Criminal History Review: the standard form and the abbreviated form. The standard form requires that the individual provide details such as their name, address, social security number, and date of birth. The abbreviated form requires only the individual’s name, date of birth, and signature. Both forms include language that explains what information will be obtained, how it will be used, and how long it will be retained. The form also provides the individual with the right to decline to provide consent and to receive a copy of the results of their criminal history review.
