Nevada REQUEST for NON-IDENTIFYING INFORMATION Adoptee-Adoptive Parent (Electronic) is a form used by an adopted person or adoptive parent to request non-identifying information from the Nevada Department of Health and Human Services. This form is available online as a digital, electronic document. It is used to request non-identifying information about an adoption, such as the adoptive parents' occupations, education levels, religious background, and physical characteristics of the biological parents. The requestor can also provide a medical history if available. The information provided on this form is used to help the adopted person or adoptive parent gain access to additional non-identifying information that is not available through the adoption file. The Nevada Department of Health and Human Services offers two types of Nevada REQUEST for NON-IDENTIFYING INFORMATION Adoptee-Adoptive Parent (Electronic): 1. Adopted Request and 2.
